Transitions Health Clinic
Forging a Pathway from Prison and Jail to a Health Home
Contra Costa Health Services is a proud partner in the national Transitions Clinic Network for those recently released from incarceration, assisting individuals with successful and healthy re-integration into their lives and communities.
The Transitions Clinic provides a primary health/medical home for returning community members with chronic medical conditions, including HIV and substance use, which are commonly present during incarceration. Upon release, returning community members face an increased risk of hospitalization and death without timely and appropriate healthcare.
We welcome those leaving jail or prison to the Transitions Clinic – no judgment, just high-quality healthcare. In addition to a doctor and other medical staff, the Transitions Clinic includes a community health worker with a history of incarceration as part of the clinical team.
The clinic offers returning community members the following:
- Primary care
- Cancer screenings
- Diabetes management
- Treatment for drug and alcohol addiction
- Mental health support services
